BECOMING A PATIENT

If you are uncertain whether hospice is right for you, we will gladly perform an evaluation of the patient and advise you if hospice would be an appropriate option now or at some later date. Patients can begin to receive hospice after being referred by their physician or the patient or family can call hospice themselves. If the physician makes a referral, we will contact the patient and family to discuss hospice care. If the patient or family makes the referral our staff will contact the physician and receive an order for hospice care. After the referral is made our staff will contact the patient and family to begin the admission process.
