PLAN OF CARE

Hospice will create a Plan of Care for each patient. Every plan is individualized and is a result of collaboration of the patient and family, hospice staff members, the hospice medical director and attending physician.

The Plan of Care includes the wishes of the patient and family regarding all aspects of hospice care, including management of pain and other symptoms. Hospice believes in treating the patient with respect and dignity, and allowing them to have a voice in their treatment plan.
