Hospice care involves a team approach to expert medical care, symptom management, and emotional and spiritual support specifically tailored to the patient's needs and wishes. Our team consists of:

The Patient and Family
Physician directed services by the Hospice Medical Director in conjunction with the patient's physician
Nurse-coordinated care in the place of residence setting
Nurse Practitioners to make home visits
Medical Social Work and counseling services
Chaplain services for spiritual support
Pharmacy Services for collaboration in symptom management
Certified Nursing Assistants for assistance and personal care

Bereavement Counselors and Volunteers to provide support up to 13 months after the loss of a loved one
On call support by our Registered Nurses 24 hours a day, 7 days a week
