Considered to be the model for quality,
compassionate care at the end-of-life, hospice care involves a team
approach to expert medical care, pain management, and emotional and
spiritual support specifically tailored to the patient’s needs
and wishes. Our team consists of:
- Patient and family
- Physician directed services by the Hospice Medical Director
in conjunction with the patient's physician
- Nurse-coordinated care in the place-of-residence setting
- Many of our patients live in nursing homes - assisted living
facilities.
- Medical Social Work and counseling services
- Chaplain Services upon request for spiritual support
- Certified Nursing Assistants for assistance and personal care
- Volunteers for a variety of services including companionship
and respite for caregivers

- Bereavement Counselors and Volunteers to provide support up
to 13 months after the loss
- On-call support available 24 hours a day – 7 days a week
